CAMPBELL'S LFE "GAME PLAN FOR GOOD HEALTH" SWEEPSTAKES

  1. How To Enter: The LFE "Game Plan for Good Health" Sweepstakes ("Sweepstakes") begins on 12:01 AM ET 12/21/06 and ends 11:59 PM ET 5/31/07 ("Promotion Period"). Go to www.gameplan4health.com anytime during the Promotion Period, click on the sweepstakes icon, and complete the entry including your name, complete mailing address, and daytime phone number, email address, along with the name, city and state of the school to receive prize if your entry is chosen as a prize winner. The school designated on the entry, to be eligible to win must be a public or private elementary school, or accredited home school association with any of grades K to 5 for children aged 6 to 11, located in the 50 United States (including the District of Columbia and U.S. military installations) and if selected as a prize winner must be registered in the Labels for Education program at time of award. Once information has been entered, check the box to confirm you have read the official rules and that you are 18 years of age or older, click "submit" and you will be entered. Limit one entry per person/email address during the Promotion Period.

    Sponsor assumes no responsibility for incorrect or inaccurate entry information, human error or technical malfunctions, lost/delayed data transmission, omission, interruption, deletion, defect, line failures of any computer equipment or software, or any combination thereof. All entries become the property of the Sponsor and will not be acknowledged or returned. Entries not complying with all rules are subject to disqualification.
  2. Prizes: (1) Grand Prize: 1,000,000 Labels for Education™ ("LFE") Bonus Points Certificate to be awarded to the school named on the winner's entry. ARV: $11,500/ea. (10) First Prizes: 10,000 Labels for Education™ ("LFE") Bonus Points Certificate to be awarded to the school named on the winner's entry. ARV: $115/each. (1000) Second Prizes: 100 Labels for Education™ ("LFE") Bonus Points Certificate to be awarded to the school named on the winner's entry. ARV: $1.15/each. Approximate Retail Value (ARV) of all prizes offered: $13,800. Odds of winning depend upon the number of eligible entries received during the promotional period. Limit one prize per person, family or household. All prizes will be awarded as long as there are a sufficient number of eligible entries. LFE Bonus Points Certificates can only be redeemed by eligible schools via proper LFE procedures through Campbell's LFE program.   3. Drawing/Notification: A random drawing to award all prizes offered, will be conducted on or about 6/15/07 under the supervision of Marden-Kane, Inc., an independent judging organization, whose decisions are final on matters relating to this Sweepstakes. Potential winners will be notified by mail and/or email on or about 6/18/07 or at a reasonable time thereafter and asked to confirm the name and address of the school that will be receiving the LFE Bonus Certificate on behalf of the winner, along with the LFE Account number and LFE coordinator name and phone number. Within 4-6 weeks after this information has been received, an LFE Bonus Certificate will be mailed to the verified entrant/winner. The winners are responsible for delivery of the LFE Bonus Certificate to the school named on their entry and school must redeem the LFE Bonus Certificate via proper LFE procedures. Within 6-8 weeks after redemption by school, a credit will be put in the winner's school's account and an acknowledgement letter confirming same will be sent to the winner and school. Potential Grand and First Prize winners must also execute an Affidavit of Eligibility/Liability and where legal, Publicity Release. These documents must be completed, signed and returned within fourteen (14) days from date which the documents are sent to a potential winner. Should a winner decide to decline the prize for any reason whatsoever, or fails to claim his/her prize or to sign and return any required document within the required time frame, or fails to meet any eligibility requirement herein, Sponsor shall have no further obligation to that potential winner, and an alternate winner will be selected. Return of any prize notification as undeliverable may result in disqualification and selection of an alternate winner.
  4. Prize Restrictions: By accepting a prize, winners agree to the use by Sponsor and its designees of their names and/or likenesses for advertising, promotional and other purposes, worldwide, and in perpetuity in any media, now known and hereafter devised without additional compensation except where prohibited by law. Prizes are not transferable and no substitutions or cash awards allowed, except by Sponsor, in which case a prize of equal or greater value will be awarded. Sponsor is not responsible for Bonus Points Certificates that may become lost or missing, once awarded to winners. All taxes, if any, are the sole responsibility of the winner. By entering, entrants agree to be bound by these rules and to release, indemnify and hold harmless, Sponsor, Promoter, subsidiaries and affiliates, their respective officers, directors, employees, agents from any and all claims or liability in connection with this promotion or acceptance, use or nonuse of any prize.
  5. Internet: If for any reason this sweepstakes is not capable of running as planned due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of this sweepstakes, the Sponsor reserves the right at its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the sweepstakes. Sponsor is not responsible for any problems or technical malfunction of any computer on-line systems, servers, or providers, computer equipment, software, failure of any email or entry to be received by Sponsor on account of technical problems, human error or traffic congestion on the Internet or at any Website, or any combination thereof, including any injury or damage to participant's or any other person's computer relating to or resulting from participation in this sweepstakes or downloading any materials in this sweepstakes. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON (S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW. Online entry can only be made via 1 email address per person. In the event of a dispute as to the identity of a winner based on an email address, the winning entry will be declared made by the authorized account holder of the email address submitted at time of entry. "Authorized account holder" is defined as the natural person who is assigned to an email address by an Internet access provider, on-line service provider or other organization (e.g., business, educational, institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address.
  6. Eligibility: Open to legal residents of the US (including the District of Columbia and U.S. military installations), who are 18 years of age or older at time of entry, except employees or agents of Sponsor, Promoter and their subsidiaries, divisions, affiliates, advertising agencies, & members of their immediate families. If prize is won by a minor, it will be awarded in minor's name to their parent or legal guardian who must sign all required documents. Sponsor reserves the right to verify eligibility qualifications of winner. Schools named on winning entries must be registered in the Labels for Education program at time of award. Void in Puerto Rico & wherever prohibited or restricted. Subject to all federal, state & local laws & regulations.
